Diabetes Education Program
The Vaughan Community Health Centre, Diabetes Program provides free education for adults living with Type 2 Diabetes. Our vision is to promote a healthy community by improving our clients’ quality of life and well-being. We offer both individual counselling and group sessions with Registered Dietitians and Registered Diabetes Nurse Educators. In VCHC, we believe that improving knowledge and empowering clients to self-manage their diabetes can reduce their risks of complications.

To register for Individual Counselling or Group Sessions call Natalie at (905) 303-8490. Group session times may vary. Please check the upcoming group session for more details. Individual counselling requires physician referral. Referral forms can be downloaded from the website. Services provided under individual counselling can be found under services offered.

Monday, Thursday, Friday 9am-5pm
Tuesday, Wednesday 9am-8pm
